Why do ApoB and LDL-C diverge? The most common driver is metabolic syndrome and hepatic insulin resistance. When serum triglycerides rise, cholesteryl ester transfer protein (CETP) facilitates an exchange: triglyceride molecules are transferred into LDL particles in exchange for cholesteryl esters.

These triglyceride-enriched particles are subsequently hydrolyzed by hepatic lipase, stripping away core lipids and creating small, dense LDL (sdLDL). Because each small dense particle carries far less cholesterol per particle, total LDL-C drops—misleadingly reassuring both patient and clinician—even as the absolute number of atherogenic particles (ApoB) skyrockets.
